• The owner of the company makes the decision to offer health insurance to his/her employees.
• If your employer/business does not offer health coverage or it is too expensive, consider an Individual and Family plan

• have at least 2 eligible employees
• be able to provide the following documentation
  • Statement of Info OR
  • Current Schedule K-1 OR
  • Articles of Incorporation OR
  • Current Schedule C (sole-proprietor)
• pay 50% of the lowet cost employee only premium

Other Considerations

• Generally 75% of eligible employees must participate in the plan offered.
• The employer is NOT required to pay for dependant coverage
